Our office building is located one mile south of Adventist HealthCare Shady Grove Medical Center and near the Slaoui Center for Vaccines Research (GSK) building.
Appointments and Walk-in Patients
Patients are encouraged to make advance appointments for medical services, this allows our staff and medical teams to review your information prior to your visit and efficiently prepare for your arrival.
Walk-in patients are always welcome; however, they may experience longer waits since patients with appointments normally take priority.

New patient registration forms are to be completed on arrival of first visit via electronic pad.
Please bring completed forms to your appointment and give to our front desk staff.

If you are calling for test results or for prescription, please press 4. (Please contact your pharmacy directly and ask that they send us an electronic refill request. Electronic refill request will be processed within 2 business days. Please allow up to 2 business days to process all requests received by fax or phone. Please contact your pharmacy directly to check on the status of your request. For all other prescription-related issues or for medical questions for one of your physicians, your call will be return within 1 business day. our may also use our Patient Portal to contact us.
