Home-Based Primary Care Nurse Practitioner
Sara has been providing home-based primary care since joining Montgomery Medical in 2016. She specializes in both transitional care following hospitalization and ongoing primary care for homebound patients.

Jennifer Eisen, CRNP
Family Nurse Practitioner
Jennifer is our newest addition to the home-visiting team, bringing board certification and seven years of registered nursing experience to patient homes throughout Montgomery County.
