Sara has been providing home-based primary care since joining Montgomery Medical in 2016. She specializes in both transitional care following hospitalization and ongoing primary care for homebound patients.

Family Nurse Practitioner
Jennifer is our newest addition to the home-visiting team, bringing board certification and seven years of registered nursing experience to patient homes throughout Montgomery County.

Adult-Gerontology Nurse Practitioner
Maxim is an Adult-Gerontology Nurse Practitioner with extensive experience providing comprehensive primary, preventive, and urgent care to adult patients. He has cared for patients in outpatient practices, urgent care centers, hospital systems, and home-based settings, delivering compassionate, patient-centered care with a focus on prevention, education, and long-term wellness.
